BIOGRAPHICAL DATA FOR PROMOTION AND TENURE REVIEW
FORM INSTRUCTIONS

Form Instructions

  1. Last Name – Enter the last name of the individual.
  2. First Name and Initial – Enter the first name and initial of the individual. No nicknames.
  3. Rank and Title – Enter the exact Rank and Title of Position (i.e., Associate Professor, History).
  4. College – Enter the name of the College.
  5. Department-Division-School – Enter the name of the Department, Division, and/or School.
  6. Location of Residence – Enter the Location of Residence (i.e., University Park, Altoona, etc.).
  7. Academic Training Table –
    1. Row 1 – Cell 1 - Enter the Name, City, and State of first undergraduate institution. Cell 2 – Enter Major Subjects. Cell 3 – Enter Minor Subjects. Cell 4 – Enter the degree received and date received.
    2. Row 2 – Cell 1 – Enter the Name, City, and State of second undergraduate institution (if applicable). Cell 2 – Enter Major Subjects. Cell 3 – Enter Minor Subjects. Cell 4 – Enter the degree received and date received.
    3. Row 3 – Cell 1 – Enter the Name, City, and State of Graduate School. Cell 2 – Enter Major Subjects. Cell 3 – Enter Minor Subjects. Cell 4 – Enter the degree received and date received.
    4. Row 4 – Cell 1 – Enter the Name, City, and State of Other Institution. Cell 2 – Enter the Major Subjects. Cell 3 – Enter Minor Subjects. Cell 4 – Enter the degree received and date received.

  8. Professional Status – Enter other degrees or licenses held (i.e., Law, CPA, etc.).
  9. Honorary Degree(s) – Enter any honorary degrees and the issuing institution.
  10. Occupational Record Table –
    1. Row 1 – Cell 1 – Enter the most recent previous employer with City and State, including U.S. Military. Cell 2 – Enter work performed (if teacher, list subjects taught). Cell 3 – Enter the Rank or Title of this position. Cell 4 - Dates (Enter from and to dates).
