General University Reference Utility
To outline the University's policy regarding injuries occurring on all University owned, leased or otherwise-occupied property, the reporting of those incidents involving persons other than employees within the scope of their employment, and to provide mechanisms to document such occurrences for appropriate handling.
It is the policy and intent of the University to render emergency assistance to anyone who is injured or stricken with sudden illness while on University property. The University has an Emergency Medical Services (EMS) system in place to respond to medical emergencies, which is activated by calling the local emergency number, such as 911. This EMS system also includes assistance that may be provided at the work unit or via transport to a medical care facility.
After the necessary assistance has been provided, an immediate report of the circumstances should be made as specified below.
- At University Park:
- Injuries shall be reported to the Safety Officer or their designated representative, who shall submit the Risk Management Incident Form to the Risk Management Office, 227 W. Beaver Ave., Suite 103, Rider Building, State College, PA 16801, fax: 814-865-4029. University Police and Public Safety, if called to the scene, shall contact Environmental Health and Safety (EHS) and the Risk Management Office (814-865-6307) if the injury or illness may be attributed to the University environment.
- University Ambulance Service shall submit a de-identified list of incidents occurring at large events to the Risk Management Office.
2. At All Other Locations:
- Accidents shall be reported to the Director of Business Services (or at facilities other than a campus, the location manager) or their designated representative using the Risk Management Incident Form.
- The Director of Business Services or location manager shall submit the Risk Management Incident Form to the Risk Management Office, 227 W. Beaver Ave., Suite 103, Rider Building, State College, PA 16801, fax: 814-865-4029. In addition, the Director of Business Services or location manager, or their designee, shall immediately contact EHS (814-865-6391) and the Risk Management Office (814-865-6307) in the event the injury or illness could be attributed to the University environment.
