Form Instructions

1. Select the date the Guest Presenter will be selling merchandise from the drop-down calendar
2. Enter the Sponsoring Area of The Pennsylvania State University (must match the University Guest Presenter Agreement)
3. Enter the name of the Guest Presenter, Lecturer, and/or Speaker (must match the University Guest Presenter Agreement)
4. Describe the item(s) the Guest would like approval to sell (be specific)
5. Enter the name of the Guest
6. The Guest must sign the form
7. The Guest must enter the date the form is signed
8. Enter the name of the University official approving the form
9. The University official must sign the form
10. Enter the title of the University official approving the form
11. Enter the date the University official signs the form
