    Principal Investigator Request

  1. Enter the Business Area Name
  2. Enter the Business Area number as assigned in SIMBA
  3. Enter the Principal Investigator(s) name(s)
  4. Enter the name of the grant
  5. Enter the grant number
  6. Enter the sponsor program number
  7. Enter a description of the equipment
  8. Enter the total approximate value of the equipment
  9. Was sponsor approval granted for this procurement via the proposal and reflecting in the final award equipment budget category? Select Yes or No. If prior approval was not obtained, attach sponsor approval documentation with this request.
  10. Enter name of individual to be responsible for requested items
  11. Enter the name of the building where the equipment is located
  12. Enter the room number where the equipment is located
  13. Business Area Inventory Certification - This section must be completed for all property acquisitions, with federal funds, valued at $50,000 or greater:

  14. Select the appropriate result of screening
    • Check Box 1 - A similar item, available/suitable for shared use, was found to exist at the following location:
      • Enter the department name where the similar item has been located
      • Enter the PSU Inventory tag number of the similar item
      • Enter the building name where the similar item is located
      • Enter the room number where the similar item is located
    • Check Box 2 - Items similar to the equipment described above are listed in the Business Area Inventory but are NOT available/suitable for shared use because:
      • Provide an explanation as to why the equipment is not available or unsuitable for shared use
      • Enter the department name where the not available/unsuitable item is located
      • Enter the PSU Inventory tag number of the not available/unsuitable item
      • Enter the building name where the not available/unsuitable item is located
      • Enter the room number where the not available/unsuitable item is located
    • Check Box 3 - No such item, or substantially similar item, is listed in the Business Area Inventory
  15. The budget assistant (acquisitions between $50,000 and $99,999) certifying the search results must provide either a hand-written signature or provide their digital ID
  16. If the budget assistant provided a hand-written signature, enter the date this information was certified. If a digital ID was provided, skip this field
  17. The budget executive (acquisitions over $100,000) certifying the search results must provide either a hand-written signature or provide their digital ID
  18. If the budget executive provided a hand-written signature, enter the date this information was certified. If a digital ID was provided, skip this field
  19. Office of Budget and Finance, Finance Office Certification - This section must be completed, in addition to the above section, for all property acquisitions, with federal funds, valued at $100,000 or greater.

  20. Select the appropriate result of University-wide screening
    • Check Box 1 - A similar item, available/suitable for shared use, was found to exist
      • Enter the Business Area name where a similar item has been located
      • Enter the department name where a similar item has been located
      • Enter the PSU Inventory tag number of the similar item
      • Enter the building name where the similar item has been located
      • Enter the room number where the similar item has been located
    • Check Box 2 - Items similar to the equipment described above are listed in the University-wide Inventory but are NOT available/suitable for shared use because:
      • Provide an explanation as to why the similarly identified equipment described above are listed in the University-wide Inventory but are NOT available/suitable for shared use
    • Check Box 3 - Not such item, or substantially similar item, is listed in the University Inventory
  21. The University Property Manager certifies the search result by providing a hand-written signature or providing their digital ID
  22. If the University Property Manager provided a hand-written signature, select the date this information was certified by selecting the appropriate date from the drop-down calendar. If a digital ID provided, skip this field
