PROMOTION AND TENURE LOG OF EXTERNAL LETTERS

Form Instructions

  1. Name - Enter the full name of the individual. No nicknames.
  2. Title - Enter the title of the individual (i.e., Associate Professor, History).
  3. Candidate For: - Check this box if the individual is being considered for Tenure.
  4. Promotion To: - If the individual is being considered for promotion, select either Associate Professor/Librarian or Professor/Librarian check boxes.
  5. Log of External Letters Table - The table consists of five columns and ten rows
    1. Column 1 - Enter the Name and Title of the Writer
    2. Column 2 - Enter the Writer's Employer
    3. Column 3 - Enter the Date Letter Requested
    4. Column 4 - Enter the Date Letter Received
    5. Column 5 - Enter the Date of Entry into Dossier
